NOTE: When filling out the entry fee portion of the form please ensure the total fees are listed; i.e. if you are entering 3 days and the fee per day is $22.00 then you must enter $66.00. You must also add the additional fees for additional classes i.e puppy sweepstakes. DO NOT add in catalogue fees or listing fees or processing fees - the form will calculate these into the total. You will get back a confirmation of your total once you hit the submit button - check to make sure it is correct. If it is not correct email the show secretary - DO NOT hit the back button on your browser and resubmit the entry. 

Please ensure all fields are filled in or the entry will pop up an alert box requesting the missing information. If you are unsure about entering your visa or mastercard number online simply insert in the number field "on file" and then either email the show secretary with the number or phone. Please make sure you enter your email address correctly at the bottom of the form if we need to contact you.

The default value for "Is this a puppy" is yes - make sure you change that if necessary

If you are going to enter a dog in the pre-registration program that is a "listed dog", you must select listed from the drop down menu and in the registration number field you must enter N/A for not applicable

With the pre-registration form you must still select the days to be entered, the class or classes to be entered and fill in your credit card information. Please double check the information to ensure its accuracy.

On the lines that require a signature - please just TYPE in your name.

EXHIBITOR NOTE: Once you have submitted your entry you will receive an email with a copy of your entry form for your records. Double check to make sure all the information is correct. If you have to make changes please email the Event Secretary directly - do not email CompuPets unless it is one of our events.

Send a follow up note to the show secretary to ensure your entry was received.
